"Adopt a Nursing Home Patient" Program

This program is dedicated to improving the quality of life in nursing homes by providing patients with companionship, emotional support, and socialization. The Dorotha C. White Foundation encourages involvement by the community to help the elderly and improve the quality of life in nursing homes. The program applies to both skilled nursing facilities (SNFs) and assisted living facilities.

Already a Part of the "Adopt a Nursing Home Patient" Program?

If you are already a part of the Adopt a Nursing Home Patient Program, you may downlod the time sheet by clicking the link below. The time sheet is in a Microsoft Word document, so you will need Microsoft Word or a compatible editor to view and/or modify the document. Instructions for downloading and returning the time sheet are as follows:

  1. Click on the link below. If you get the option to "Save" or "Open" the document, then choose to "Save" it and save it to a location on your hard drive that you will remember (if you do not get this option and the document automatically opens then you will need to go to the "File" menu and choose "Save as..." and save it to a location on your hard drive).
  2. Open the document that you just saved.
  3. Fill out the time tracking information as needed.
  4. Save the document (you can either save over the original or choose "Save as..." if you want to keep copies for future reference).
  5. Close the document, create a new e-mail addressed to srhoads@dcwhitefoundation.org and attach the completed document to the e-mail and send.
